This section contains some files that may be useful. Most of the files are PDF format. 

A word about entry forms. You can use a service like Oaklines but now that the AKC & UKC forms are fillable and savable by the Adobe Reader it does not make much sense to use them unless you are going to do an online entry with them. It is just one more place to leave your personal information and another password to remember. Read about how I handle entry forms.

AKC Agility computer fillable only form (24C)

Same as the one below with 24C as an option in the jump height field for Regular classes. Otherwise no difference. Both 24C and 26” jump heights are available so you can use it for trials before and after 1/2/2018.

AKC Agility computer fillable only form (rev2)

This version of the AKC entry form includes options for the premier classes. Note, this form can only be filled in by computer. It uses things like drop down menus for selections instead tiny check boxes. This form will handle up to a five day trial and includes all eight possible classes that can be entered. The form will calculate the total fees for the entry. 

You can save copies of the form with the static information about each dog. Then to enter a trial, you just enter the club name, the per run fees, the first date of the trial and check the classes entered. You can then print and mail the entry form. I suggest that you do a "Save As” and save the form with a different name so you have a record of your entry.

Across the top of the form are the Fees: These are the per run fee for the first, second, etc. runs. You only need to account for the maximum number of runs that you will run in a day with a dog. The form will compute the total fee as you check or un-check the boxes. Enter the first date of the trial in the first date box. The remaining dates will be calculated and the the day of the week computed and displayed. Note, the fee boxes will not be printed.

Rev2 fixes a problem where the day of the week was not always being computed correctly.

AKC Agility entry form 7/2015

Updated for the Premier Classes. This entry form is fillable and can be saved with your default information by the Adobe Reader. It has space in the top for the club name and up to four trial days. The link is to the form on the AKC web site. If the link breaks, let me know and I will re-link it.

AKC agility event schedule calc.xlsx

This is a basic event schedule calculator spreadsheet for AKC trials. It lists the events, the date target and the number of days until the target date. The number of days is conditionally formatted with backgrounds of

  • White for > thirty days before the target date
  • Yellow for > 7 days before the target date
  • Orange for 1 to 7 days before the target date
  • Pink for less than one day before the target date

As a spreadsheet, it is pretty easy to modify the calculations to suit your preferences.

2013-07 Manatee RV form fill&save.pdf

New RV form for Manatee River Fair in Palmetto. All sites now have water and there is no longer any pricing for sites without water. You can fill this form in with your static information and save it. Then just fill in the dates, print and mail.
